AFFIRMO
Summary
AFFIRMO (Atrial Fibrillation Integrated Approach in Frail, Multimorbid, and Polymedicated Older People) is a project aiming to improve the care of older adults living with atrial fibrillation and multiple long-term conditions. The project uses a “holistic Atrial Fibrillation Better Care (ABC) model” to bring together patient empowerment, clinical decision support, and digital health tools. Central to this is a multilingual mobile app for patients and a web portal for clinicians, enabling better communication, monitoring, and shared decision-making.
Platform
Native app for iOS and Android and a web interface for clinicians
Funders
AFFIRMO is funded under Horizon 2020 (approx. €6,000,000). Partners span universities, research institutes, and organisations across Europe.
Services
- Digital health research
- Co-design
- UI/UX design
- Software development
- Technical project management
- Quality assurance
Our role & approach
- Ran co-design sessions with patients and clinicians to shape the app and web portal.
- Developed a multilingual patient-facing app supporting education, empowerment, and medication adherence monitoring.
- Built a clinician web interface that displays patient-reported data, including prescription information, adherence tracking, and engagement levels.
- Ensured the system supported shared decision-making by linking the patient and clinician views.
- Supported accessibility and usability, tailored to an older population with multimorbidity.
- Iteratively develop features and user interface, based on user feedback.
What we built: Mobile app
- A multilingual mobile app (available in all study languages) for patients living with atrial fibrillation and multimorbidity.
- Provides tailored educational resources and empowerment tools to support self-management.
- Includes features to help patients track prescriptions, medication adherence, and symptoms, sharing this data with their care team.
- Designed to improve health literacy and engagement, supporting the project’s goal of empowering patients as active participants in care.

What we built: Clinician portal
- A secure web-based platform for clinicians participating in the study.
- Displays patient-reported data in a structured way, including prescription and adherence information.
- Allows clinicians to monitor patient engagement with the app, informing shared decision-making during consultations.
- Integrates with the holistic ABC care pathway, supporting coordinated care for patients with AF and multimorbidity.
Outcomes & impact
- Delivered a working dual-platform solution (mobile app and clinician portal) now used across multiple European sites.
- Supported multilingual deployment, with the mobile app available in Italian, Spanish, Danish, Romanian, Bulgarian & Serbian.
- Enhanced opportunities for shared decision-making by directly linking patient data to clinician dashboards.
- Positioned AFFIRMO as a model for integrated care using digital health tools at scale.
“For 20 years now the Digital Health Software team have supported my research projects by delivering high quality software to specification and budget. They provide dependable operational support across the whole study lifecycle from requirements capture to data analysis.”
