In this study, we examined the assessment and management of suicide risk in primary and secondary care.

Interviews with 198 GPs and an examination of 291 case records for patients who had died by suicide within 12 months of contact with mental health services revealed the overall agreement in the rating of risk between primary and secondary care services was poor. Higher risk was usually identified in secondary care.

In primary care, depression, care setting after discharge, suicidal ideation at last contact and a history of self-harm were associated with a higher rating of risk. Policies and training around suicide risk assessment and management were inconsistent across GP practices.
